Frequently Asked Questions About Death
Records
- Question: How can I receive a Certified or Non-certified
copy of a Death Record?
- Answer: Provide a signed,
written request with the following information:
- Name of individual
- Your relationship to the decedent
- Date and place of death
- Reason you are requesting a certificate
- Submit a photo copy of a photo ID (ie: driver's license,
State ID, etc) of yourself.
(If your ID does not have your signature on it, then the
signature on your request needs to be notarized.)
- Daytime phone number or email address (in case we have
questions)
- Submit $3.00 per Certified copy requested, or
$1.00 per Non-certified copy requested. No out
of town checks accepted. For out of town requests, submit
a money order, certified check or cash for the fees. Be
sure to provide a return address.

Death Records are sent back via regular postal service mail
within 24 hours of receiving the request.
