Frequently Asked Questions About Death Records

 

  • Question: How can I receive a Certified or Non-certified copy of a Death Record?

  • Answer: Provide a signed, written request with the following information:
    • Name of individual
    • Your relationship to the decedent
    • Date of Death
    • Reason you are requesting a certificate
    • Submit a photo copy of a photo ID (ie: driver's license, State ID, etc) of yourself.
      (If your ID does not have your signature on it, then the signature on your request needs to be notarized.)
    • Submit $3.00 per Certified copy requested, or $1.00 per Non-certified copy requested. No out of town checks accepted. For out of town requests, submit a money order, certified check or cash for the fees. Be sure to provide a return address.

Death Records are sent back via regular postal service mail within 24 hours of receiving the request.
